Place your foot on the floor; put the tape measure under the ball of your foot and measure the circumference, ensuring that you are measuring your foot at its widest point. Enter this measurement (in centimetres) in the blank field directly below.

Still keeping your foot on the floor, put the tape measure under the arch of your foot and measure the circumference, ensuring that you are measuring over the bridge of your foot (ie. at the highest point of your foot). Enter this measurement (in centimetres) in the blank field directly below.
Put your left foot on an A4 sheet and (using a black pen) draw around the contour of your foot.
Take the A4 sheet that you have just used and measure the length of your foot, from the heel to the tip of the longest toe. Enter this measurement (in centimetres) in the blank field directly below.
Scan the A4 sheet that you have just used, then click Browse to locate the scanned PDF document and – once you have found it – click Open to attach it.
